The purpose of this project is to allow you to submit
your family groupsheets to the Wisconsin Archives.

Be sure to use the submission form (or alternate email form) below
as that is the only format that will be accepted for this project.

Instructions for Submitting Your Family Groupsheet

The form below is made for a family of 15 children. It is only 2 generations since there is no simple way to do more than that and store it. To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname. We hope that you will also submit a separate chart on the mother's family. If there is more than one marriage, submit a second family groupsheet and make a notation in the documentation field that states this is an additional marriage.
