Declaration for Invalid Pension

To be executed before a Notary Public, Justice of the Peace, or any officer competent 
to administer oaths.

State of Pennsylvania, County of Chester, SS:

     On this 11 day of July, A. D. One thousand eight hundred and ninety-___, personally 
appeared before me, Alfred Rupert - a Justice of the peace, within and for the County 
and State aforesaid John H. Baily, aged 50 years, a resident of the borough of West 
Chester, County of Chester, State of Penni’a, who, being duly sworn according to law, 
declares that he is the identical John H. Baily , who was enrolled on the 20th day of 
August, 1862, in Co. K 124 Regt. Pa. Vols. as corpiral in the War of the Rebellion and 
served at least ninety days, and was Honorably Discharged at Harrisburg Pa on the 17th 
day of May, 1863.  That he is partially unable to earn a support by reason of loss of  
right arm at shoulder.   That said disabilities are not due to his vicious habits, and 
to the best of his knowledge and belief permanent.  That he has applied for pension under 
application No.__.  That he is a pensioner under certificate No.__.  (If a pensioner, the 
certificate number only need be given.  If not, give the number of the former application, 
if one was made.)  That he makes this declaration for the purpose of being placed on the 
pension roll of the United States, under the provisions of the Act of June 27, 1890.

     He hereby appoints, with full power of substitution and revocation,
Alfred Rupert of West Chester Pa his true and lawful attorney to prosecute his claim, the 
fee to be Ten Dollars, as prescribed by law.  That his post-office address is West Chester, 
County of Chester, State of Penni’a.
